Nurse home visits—at no charge

Johnson County senior adults who need some help with health-related issues at home should be aware of a free service offered by the Johnson County Health Department.

Through the Outreach Nurse Home Visiting Program, registered nurses provide assistance to seniors with limited support systems, multiple health issues, and difficulty performing daily activities.

No acute care is provided, but services offered include:

  • Physical assessment
  • Blood pressure checks
  • Blood glucose checks
  • Nutrition status
  • Fall risk assessment
  • Environmental safety assessment
  • Education about disease conditions
  • Medication education and management
  • Health promotion
  • Community resources
  • Caregiver support

Visits are scheduled depending on client needs, and there is no fee for services.
