Organizations or individuals interested in offering this six-week program are invited to apply to send teams of two. Applicants will be notified by e-mail when their application has been received. Applicants accepted into the training will be screened and notified by e-mail. Hotel information and the program location will be provided in the acceptance notification.

Each Kansans Optimizing Health Program held in the community must be led by two trained leaders, who should be living with a chronic condition. At least one of the two leaders must be a "peer leader" rather than a health or social service provider.
Organizations sending participants must commit to the initial four days of training plus coordinating and providing at least two six-week community programs each year, the first within 90 days of training.
The leader training is offered at no charge; mileage will be reimbursed, as will some hotel expenses. Participants will receive the leader manual and the Living a Healthy Life with a Chronic Disease book.
